The Conferencing Service is designed for those who wish to initiate and schedule a conference call that includes out of town callers, and pay for the associated charges. The person requesting this card is responsible for the security of the card. Instructions will be included when your card is received.

I acknowledge that if a card is issued to me, I will be the official "host" for all conference calls made using this card. As host I am responsible for all charges associated with this card. In addition, I am responsible for initially conducting the conference calls. Only official business calls will be made using this conferencing card. I will accept full responsibility for all conference calls charged to this card, and I will cancel the card if I leave the University of Kansas. * indicates required information
